HNHS Regulated Medical Waste Training Request Form

The U.S. Department of Transportation Hazardous Materials Regulations (49 CFR Part §172.704) requires hazmat employees to be trained. Included in our service to you, HNHS Waste offers an online interactive training course which covers many requirements for Regulated Medical Waste (Infectious Waste), segregation, packaging, handling, and transportation. Per regulations, this training is required every three years.

Please provide: Company name/address; contact name/phone number; as well as the name and e-mail address of each employee who will need to be trained. If you have any questions, please don’t hesitate to give us a call at (269) 329-3200.

FEES – This training is free for HNHS customers. If you are not a current customer, we do offer training for a fee of $25 per person. Please fill out and submit the form below and a HNHS Customer Service Representative will contact you to make payment arrangements.

NOTE – Each employee must have a different email address. Email addresses may not be used more than once.
